CUI problems when opening a second drawing file

When I open AutoCAD my enterprise cui, main cui and all the office custimization (lisp, menus, tool bars & shortcuts) load and work just fine. But if I create a new drawing, or open a 2nd file the cui goes screwy. Occasionally it adds a duplicate of one of the menus, and none of my customization works.

I also get the message in the comand prompt:
"Substituting [simplex.shx] for [romans.shx].
Regenerating model.
AutoCAD Express Tools Copyright © 2002-2004 Autodesk, Inc.
; error: Unknown exception occurred
; warning: unwind skipped on unknown exception
*Cancel*"

But I do not get that message the first time I open AutoCAD, only when I open additional / new files on top of the original session.

I turns out my problem was a currupt partial CUI file. I replaced the file with a new one and everything woks fine now.
