Probably an easy ? to answer but here goes...

I created this Amplifier family and it is wall hosted. When I place it into a project it does not align with the wall. What is causing this?

You have to align and lock the sketch of your amplifier with the face of the wall within the family.

See the corrected version.

I know it's an alignment tool issue.

I tried to align it when I do I get a "constraints not satisfied" error.

If you see the family I corrected, i've aligned the edges of the sketch to the wall in the family and locked it. I didn't have any message of "constraints are not satisfied" when working on it.

And before posting it, I tried to use it on a wall in my project and it worked perfect. Even changing the wall thickness will move the amplifier to the new face position.

I see how its done now.

How do I force the Annotation Symbol to align to the wall also?

Also why does MEP display the linework around the Anno symbol as randomly broken?

To force the annotation to align to the wall, follow the same rule: Align and lock things.
I aligned the annotation nested family to the wall. Now both amplificator and annotation are aligned. I have also aligned the annotation to the center of the amplificator.

Regarding the broken linework around the anno symbol, that's because your text is too big. If you edit the anno family you'll see that you text has a margin around it. Since texts are supposed to always be on top of everything, the margin of your text is on top of the linework, making it disapear and look like broken. As the wall angle changes, that text margin seems to stay always horizontal, making the break line appear on different positions around your annotation symbol.

I corrected your family again (see attached) and changed the text size. It works. However, it might not be the size you want. You may want to play with that to find the optimum size. Or use a graphic A rather than a text.

you can also go to the text properties in the annotation family and set the text to transparent instead of opaque. it will not mask out background lines under the text any more, but it also will not mask out your symbol's linework.
