Imperial Dynamic Bolt

I have altered this drawing so that I caould use it at Imperial scale (1/4", 3/8", ect) insted of Metric. I got it to work fine for 1/4" - but when I move up the scale of sizes, the washers / nut are spaced off the bolt shaft. any way that I can keep the washer / nut attached to the bolt shaft? (I copied the drawing of a Structural bolt from Mr Cory.)

what version of ACAD are you using?

Using AutoCAD 2009 (I do have 2010 - but have had some problems with it so am using 2009 for most).
