Sounds like the recipient's AutoCAD cannot locate the image file.
In the ETRANSMIT dialog, click Transmittal setups.. button, then Modify... button
In the Path options, choose, Place all files in one folder.

If that doesn't help, then it's on their side. What usually happens is they open the ZIP file, then double click the DWG inside. Windows' browser (or WinZip, or whaever they use to open the ZIP) then extracts ONLY the DWG file to a temp folder and opens that in ACad. All the other files are still only inside the ZIP - which ACad can't get to.
The way to open an eTransmit is to extract ALL the files from inside into a folder / subfolder created in the zip itself. Then double click / open the DWG file. That way all the files are available to ACad.
