Default Custom duct accessory does not break into ducts

I'm trying to create a simple Phoenix Valve as a duct accessory. I've got it set to 'break into' (I used one of the OOTB filter panels as a guide) but it won't. When I place it, the duct highlights as if it's going to attach to it, but when I actually place it, it shows up BELOW the duct. I've attached the family (2009). Any thoughts? Suggestions?

    Thanks in advance!
    Matt

it seems to work fine in 2011...but not in 2009. I know that's no help but interesting.

Yet another reason for us to upgrade (too bad we're at the mercy of the archies). :\

It looks like there weren't ebough constraints for Revit to place the valve successfully. Changed fam type from damper to breaks into. Put duct connector contraints, and some geometric constraints, loaded it, and poof. It worked.
