Does anyone know why this seems to happen alot when working with sloped pipe? I align it from both directions and then trim/extend. Not sure what is causing this problem...See image below.

I've encountered the same thing numerous times. Usually I can get it to connect correctly by deleting the elbow and the "offset", re-aligning from both sides (again), and creating a new elbow with the trim command. Sometimes changing the view orientation helps, such as creating the elbow in the section view instead of the plan view, or vice versa.
I don't know why it does that. It seems random, but typically it starts with an alignment issue.
First provide more space for bend, more than bend. set short radius bend in route preferences then try. after it is done well move the bends closer if you want
Another suggestion is to delete the weird transition and elbow and trim them back together, alternate which pipe you pick first for the trim command.
If nothing else works, delete stuff a few pipes back and just recreate them. I tell new modelers - Don't be emotionally attached to stuff that's not working, just redraw it.
