Did a REGEN after a copy/paste-leader went somewhere issue....did not change the stray leader. I did check the coordinates and the tail end X/Y on the stray matched the tail end X/Y on the original.

Post a DWG file with the issue, put notes inside on how we can replicate your issue and let's all have a real look at what you are doing. I suspect because you are using an ancient tool in an ancient version of AutoCAD, everyone here forgot it's inner workings enough to not be able to help figure out the issue.
If you mean that the values at the correct location and at the wrong location are the same then there is definitely a serious problem.
1, do as Dean says (above).
2, do a complete reinstall of acad.
3, get details of display hardware and driver.
4, get a more capable computer.

I have noticed that by using Leader from the Dimension menu does not cause stray lines, only if I QLEADER from the Dimension toolbar.
I just tried copy and paste on several drawings and all transferred fine.
I may have discovered the issues involved.
Earlier this week I had met with an engineer for project review and stamp. He suggested that I limit the use of arial text in my drawings. He felt that arial is a RAM hog and recommended using simplex text instead. Since then I have converted a vast majority of my blank masters to simplex.
I do reliaze that the computer that I am working on is "slightly" under powered, I'm still being the squeaky hinge with my boss on that issue.
I'll keep you posted if the problem pops up again..
