can't create new tool palette tools

i'm in ACA 2014, i am trying to create a new tool to add to a palette. every time I drag it out of the drawing onto the palette nothing happens. i have saved the source drawing.

Is the tool palette onto which you are trying to add a new tool editable?

Check the bottom corner of the palette, on the "tab" side. Do you see a lock icon or a refresh icon? If so, the palette is not editable, and you will not be able to add new tools to it. If you right click on the palette title bar, and choose New Palette from the context menu, are you able to add a tool to that palette (assuming that you are able to create a new palette)?
