Default Door/window family, to create or just model in place

I have a very unique, one off opening in a wall. Something I've never seen before. Think of a pass-thru window. Only this is a door. There's a leaf on the exterior and a separate leaf on the interior. The exterior has a different width and height from the interior.
I'm thinking this is one of those cases that would just be easier to put the hole in the wall, and the model the doors in place, and add some line work for what needs to show in plan.
I'd try to build one, but I've never seen anything close for me to even think about trying to get it to work.
Any ideas on a way to accomplish this in a family, or would you model this in place?
    Thanks in advance.
