    Default Dimension Placement

What magical setting made this dimension text positioning possible?

It would be wonderful for when dimension text fits between the line but added text (which I would place below the line) doesn't.

Showing the text above the line is part of the ISO standard. Perhaps creating a custom style and playing with the settings will get you what you want.

Sorry, I'm referring to using the 'First above the Line' option in the style while keeping free placement. This would enable me have the dimension figure centred above the line, with some short descriptive text below.
There are a number of instances when the dimensioned length/width/height isn't great, and the descriptive text would clash (even very slightly) with the extension lines.
This forces the text either side of the dimension extension lines.
However with First above the line enabled the descriptive text below the line wouldn't clash with the extension lines in many instances.
The example I chose to show in the initial image is actually quite a bad one to show this, but I'm sure you know what I mean.

I understand what you are trying to achieve, but, where, actually, in Inventor are you seeing this option? I cannot find it in either the Style Manager nor the RMB context menu.
