Default Family showing in plan even though it shouldn't be

So, I've been trying to figure out this family for a while without any luck. I'm creating a concentric vent to show on our walls and for some reason it's showing on top of the wall in plan, even though the elevation is set to 9'-0". The Cut plane in the floor plan is set to 4'-0" and the Top is set at 7'-6". I've attached an image of what it looks like.

Also, a side problem. I've set the origin of the family by creating two reference planes that are set to "Defines origin" (see attached pic). Unfortunately, when I go to place the family in an elevation view, it is being placed by the top-most part of the element, in this case, being the top of symbolic line as opposed to the center of the concentric vent.

    Any thoughts?

    Thank you in advance!

    Just a few thoughts....
-Are you sure it is attaching to the level you want it to? My guess is that you placed it in elevation and Revit guessed on what level it should be attached to. The only way I know to get around that is to place in in the floor plan that is associated with the level I want it to be attached to and then finalized the location in elevation.

-When you created the family did you create it from scratch? I bumped into something similar once, when I repurposed a family and some how it decided that there was a hard dimension set that threw everything off, and I could not find the dimension at all. I had to recreate from scratch.

anyway that is what bubbles to the top of my head...
