Only way I have figured this one is to do it as 2 staircases. A touch complicated though getting the levels and the display correct.

As far as I know other applications also share the inability to derive stairs that cross back over themselves...at least I know ADT used to have that problem too. To layout a spiral stair that crosses over itself we'd need a completely different way to input the design criteria. Sketching lines can't work...we'd need to supply parameters and let Revit derive the stair instead.
It would actually be kind of nice to say, "I need a spiral stair" based upon:
30" Radius
6" diameter pole
22 degree treads
max riser height of 6"
floor to floor height (level to level)
and provide the start point and let Revit work forward/upward, or the end point and let Revit work backward/downward
