Creating An Attribute Block For Revision or Title Block

Could you help me to create a simple Title block and Revision Attribute block?

I can use it but don't really know how to make one.

You could always make a separate drawing with the revisions and dates in it, xref it into your sheets so it will automatically update and all that!
