Default Unable to Bind Xref's / Some Xref's no longer there

Can someone please explain the process of binding x'refs to a dwg? I'm in the x'ref manager and I select the file and choose bind.....and it doesn't. It just keeps it's status. Also, I had a problem with another dwg. Everything was fine w/ the x'ref's attached to this particular dwg. I closed out of it and a few minutes later, after realizing I forgot something, opened the same dwg up and 2 of 3 x'ref's had disappeared. They were showing in the x'ref manager, but AutoCAD somehow just lost the paths to them. I specify the path for it and they show back up, but how'd they get lost in the first place?

    Hmm,
Problem 1, possibly nested xref? Does it return any errors on the command line after the unsuccessful binding?

Problem 2, possibly the 'save path' button needs to be hit? Did the original location move or a folder get renamed?
