Brendan and I went to great lengths to create a family to get around this one, because as you say, the out-of-the-box transition (took me a while to realise what OOTB stood for!) doesn't always work as we would want it to.
We called the family "universal duct transition.rfa"...it's a bit cumbersome but it seems to do everything you could ever want it to, ie, flat on any side, top or bottom, it can offset and transform at the same time, and its' length isn't governed by the offset angles either, meaning that it can be as short as any site constraints may require it to be. Oh, and of course, it maintains conectivity, too.
