how i can create a handrail as the picture shows. i try many time to set the baluster in the setting, but i can not get it works

Just set the grip rail to the height and offset you need in the Rail Structure of your rail's type properties... unless you're referring to something else?

On a side note, you need more risers to make that stair ADA. Risers need to be between 6" and 7", so you'd need 18 risers @ 6 2/3". I don't know, that may just be the IBC that requires that.

How can you extend the handrail longer than the toprail?

You could try modeling the actual handrail element (the piece you put your hand on) as a separate railing from the guardrail element. This gives a lot more freedom in defining how the handrail extends, turns & returns - especially if it jumps from being anchored to the guardrail construction to hard wall building construction.

i have used solid sweeps to do this
it tured exactly where I wanted..
altho is is a separate from the railing object
