This very well may be a Windows issue, but I'll ask since the problem is showing up in Revit...
In the open dialog box, most of the shortcut icons (along the left side of the dialog) are being deleted (but not all of them) on their own. It doesn't matter if we add shortcuts manually or through the File Location/Places. They'll be there for a while, then poof - they're gone.
Even a right click and "Restore Standard Folders" doesn't work.
I've attached two screen shots. One with the shortcuts deleted and another with how it should look. It's weird that it doesn't delete all the shortcuts...
