Titleblock Line Item

I want to create a small block of text in the corner of my titleblock family that has a list of about lines for a name and date that a schematic plan was approved. It would look something like a revision schedule but it isn't. I know how to do Shared Parameters. Do I have to make a new shared parameter for every line that is a different date?...like Date 1, Date 2, Date 3, Date 4, etc? I really just want to add a label to each line that the user can just type a date.

One possible solution is this:

(This is the basic idea; add the graphics, more parameters, etc)
Edit the titleblock, add a label. Make it work with a text shared parameter, something like "Schematic design approval date". Add a parameter to control its visibility. In the project, add that shared parameter as an instance, text, project parameter, that applies to "Project Information". Then, to enter the info in the label(s), go to Manage > Project information.
