why certain' Quick Access Tools' keep disappearing from the 'Toolbar'
on some occasions when Revit 12 is loaded?

I had the same issue. For awhile I kept re-adding them to the toolbar but that got old quick. Now I tend to use the keyboard shortcuts more than the toolbar and with more and more practice, it's working out to be much quicker! :)
