I'm sure this is easy but it sure as hell has me stumped!

One inch equals 25.4 millimetres, exactly.

One millimetre equals 0.03937008 inches, approximately or 1/25.4 millimetres, exactly.
Since you can't scale (in acad) by 1/25.4, scale by 10/254

Maybe take a look at the 'CAL command and using its "cvunit" option.
