There are tons of threads on this but I can't seem to find any about the same issue I'm having. I set up my Reference Line and added an angle parameter and that works perfectly. The problem occurs when I nest my Generic Annotation Handicap symbol in and try to align and lock it to the angled Reference Line. As soon as I flex the angle parameter I get an error message that Constraints are not satisfied and the wheelchair symbol is unconstrainted. Anyone have any tips on nesting?

Actually, nesting of an annotation family is not necessary if the only reason is for achieving rotation. Annotation content can be simply assigned to a workplane; the horizontal workplane of a reference line.

The annotation I'm trying to nest is a small wheelchair symbol. We show this symbol in plan on all Handicap accessible elements such as lockers or toilets. The issue is that depending on the orientation of the element the symbol could be upside-down or sideways. Any ideas on how to solve this?

The same. The symbol's workplane should be assigned to the horizontal workplane of the reference line. This avoids the align-and-lock method that is giving you errors now. There is an example of this method in this blog article:
