What is the best way to modify/represent a monolithic precast stair at the edge of a floor slab where I need concrete as shown in red in the attached image.
Can I do an inplace mass and join to the stair?
Can I adjust a property of the stair? (In looking at the notch option, it seems to only affect the top end of the stair.) Do I play with the starting level offset?

I'm using 2013 component stairs if it makes a difference

You might try using a slab edge, with a custom profile (drawn as in your sketch) attached to a floor (landing), or perhaps just extend the landing below the stair?

I'll try that and hopefully be able to join to the stairs....otherwise some linework to represent it in section correctly

The slab edge works ok....linework is in order to clean it up in section.

What might cause me the biggest issue with this method is that our floors are going to live in our consultants linked structural file. If I have to delete my floors (landings), I lose this detailing. Perhaps it wouldn't be a big deal to have these floors duplicated in the stairwell?

The question I always ask myself in these kinds of situations is, "Where and how often will this condition be documented?" If the answer is a few views, I clean the whole thing up with linework/fill regions in just those views. If it is for visualization, then I figure out a way to do it in 3D, even if it is just a hack like an in-place family. If it shows up a lot, then I spend the time to figure out a "realistic" way of doing it, like in this case the slab edge like Cliff suggests.

Here's another idea, though: start your stair one riser lower and draw the landing as the first stair. Then it cleans up exactly as you want, but you won't have much control over the thickness of that first landing as it will always be the thickness of the riser height.
