Revit Mep family lookup table

Dear reader,

I'm trying to make a family with a lookup table (.rfa and .csv). I failed multiple times (beginner). I was wondering if someone could send me a very basic family with a lookup table for me to analyse. (I'm currently working with Revit Mep 2013).
