Drawing register/issue slip

Hi All.

Long-time no see, sorry have not been on the forum for years.
Hope someone can help.

A: We want to have a drawing register showing all revision issued on every sheet. So in short the history of revisions per sheet.

Create a Shared Parameter Group (Say "Sheet Index Issue date)
Then create a shared parameter (I like Text Based) for each issue (04-07-2015 - You can rename this in the schedule later if you want, but you can't rename the parameter.
Add that shared parameter to project and apply it to sheets.
I like to fill out the parameter with an "X" for sheets issued, "R" for reference "B" for bid "I" For information.
Then you can create a schedule and list all of them.
Is that the kind of thing you're looking for?
I know there were several other threads here that talk about using this method.
