Random Callout Deletion?

We are experiencing some random callout deletion with our central file. It can be any callout, even if it hadn't been activated in a while. The Callout is deleted along with all it's notes and detail lines. We essentially have to re-created the callout.

Has anyone else experienced this? Or is it a problem with our file?
