I want to increase the size of the title text so I can read it when I plot to

Edit the annotation family. It's not a system family, so you can make it look however you like...

I got the text bigger, but still no word 'scale' in front of the 1/8"
even though I have it in the sample text that way.

Just put regular text in the family that says "scale". Sample text is just a placeholder for when you insert the label in the family.
