If you have a linear dimension type defined with a centerline pattern, what exactly controls whether or not the extension line is a centerline or not? There are some families that have a ref. plane at the center, but when I dimension to it, I do not get a centerline. Other families seem to put a centerline where I don't want it when I dimension to the edge of it or something. Any ideas?

Open the family and go into the properties of a reference plane. It can be defined as "center - elevation", "center - left/right", or "center - front/back". If it's defined as any of these it will get a centerline mark, otherwise it will not. At least that's my understanding. And you can only have one of each.
