keynote format

I'm having a problem with my keynote chart format. For some reason is is double spacing each keynote and listing each number twice. For example:

02300.A
02300.A EXISTING SOIL
02300.B
02300.B COMPACTED FILL
03300.A
03300.A CONCRETE SLAB


.....and so on down the list. Does anyone have any suggestion as to why this is happening?
