Sorry guys, we're looking in to the problem. We've gotten a bunch of responses (thanks!) but, apparently, Survey Monkey has now closed the survey. Perhaps due to overwhelming response from the fantastic enthusiasm on this site? :) I'll post when it's up and running again.
