When I am in RISA 3D and change a structural member to a size that is not in my Revit Structure model, will Revit Structure 3 add the structural member, notify me that I need to load a family, or do nothing.

To specifically address your question, here is an excerpt from that document regarding Risa 3d to Revit Structure 3, the shapes must be loaded in Revit for them to be re-imported. Please check with Risa support for more information regarding the specific behavior of the link during re-import:
RISA-3D Elements Supported for Import to Revit
Members : Columns, Beams, and Braces
Changed member shapes will be Imported back to Revit. RISA-3D shape names are mapped to Revit family type names via the shape mapping file. Updating the shape is subject to the limitation that the new shape be in the same Revit family as the original shape, and that the new shape is already loaded into the Revit project. The Redesign Lists, that are used by RISA-3D for shape optimization, can be edited by the user to limit the redesign sizes to only those shapes that are in the Revit project.
