Family drop down menu

I have loaded a family into the project (Hand rail round with extension end), but its not visible in the family drop down menu. Does anyone know the reason ?

Are you sure you didn't load the profile family for that railing? If that's the case, you'd have to create that railing type in the railing command, using the profile you loaded.

You're almost correct... The handrail extensions available on the Revit Content web site (http://revit.autodesk.com/library/html/index.html) are actually of the Baluster family type, and will only show up in the Baluster Placement section of the Railing edit dialog. They can be placed as either balusters or posts,
