Here is one of my fasteners I created. I try to avoid using rotation parameters...they never seem to work too well for me lol. its just as easy, in my opinion, to click the hotpoint and rightclick...rotate.

nope always start from stratch doing them

your thread parameter needs to be polar not liner
(see the test block in this drawing)
any of the parameters that need to rotate with the polar stretch also need to be polar. If not they will not work properly. If you try and change the parameters to polar that are associated with the look up you will have to rebuild the look up

I've noticed that you have a few actions associated with other actions (i.e.: scales)
did you do this on purpose?
Here's a fix that I found ...
There were a few actions that included other actions, so I removed it ..... and then i looked at the block a bit closer.
From what I could tell, the problem seemed to be that the base that the polar rotate was working with and the base that the array were working on were different. So I added a basepoint and that seemed to fix the issue.
Then I switched lookups to the 8g and things seemed to get back out of hand. SO I imagine this is simpyl a matter of lookup settings and that's something I'd prefer you did cuz I don't quite have the time right now.
So here's what I did and let me know if this helped at all, okay?
