I have an elevation tag that displays correct in plan, but not in a plan callout.
Any thoughts?

Did you try hitting the "show hidden elements) lightbulb, to see if maybe the circle is turned off?

Tried that...
Also tried Visibility/Graphics, the elevation's extents (whether it crossed the crop region), worksets, phases...

Maybe try placing another elevation in the view within the crop region, and see if you get the same result.

Maybe the section line of D1 do not cut the Plan Callout

I've seen that happen when the tag was hidden in the view prior to activating another elevation on that tag in another view. It's odd that the Reveal Hidden Elements tool didn't uncover the issue.

This may be entirely wrong, but it seems that the elements that are missing from the tag are 'under' the bed, so to speak. I am just transitioning to Revit myself, and this concept is a hold-over form my AutoCAD ways, but can you send the tags to the 'Front' or the beds to the 'Back'?
