Plotting in Monochrome

When plotting in Monochrome (to get prints in black and white), any layers with true colors plot in color not in black and white. All layers in index color plot in black and white. Is there a setting so that everything prints in black and white?

Probably the dither or gray scale settings in the CTB or STB file.
