When creating a curved stair, I am wanting to set a tread depth preferrably from the midpoint of the tread width.

Whenever I create it however, the dimension from midpoint to midpoint on the treads is never the correct dimension, always between .5 & 1mm out - has anyone else come across this issue?

Has anyone ever had a stair built to that tolerance would be a more relevant question

Set your dimension tolerance to 1mm when annotating the drawing. Although it's very tempting, Revit should not be treated as a 1-to-1 'building simulator'. If you get a meaningful drawing, that succesfully communicates design intent, then you win. Anything else is a bonus.
