I enter 1/8" and it offsets .1342
I enter 1" and it offsets 1.0002
I draw a line and offet it 1" and it offsets 1"

Hi,
ACAD is usually independent of the units ( " or mm etc.). There could be good possibility that by typing ( " ) with the number, you might be confusing the program. The another good explanation: using a faulty template. Try changing the template and avoid using ( " ). I did mine using a 1 offset, it comes to perfect zero upto any decimal number you name it.

Let me guess, you measured the distance using osnaps midpoint to midpoint of the parallel sides.
Should be midpoint(or nearest/endpoint) to perpendicular. The latter makes sure we are measuring the perpendicular distance.
