Is there a way to Add 3D clearances to MVparts and have the ability to turn them on or off in a drawing? In other words a visibility state for MVparts.

Can't do it with MvParts. I have only created a few parametric parts, not out of need but out of aspiration, and it seems you could have something that can function as clearance. Even then, it depends on the type of equipment you're modeling. And it can get WAY more complicated creating parametric parts.
I always created a normal solid on a clearance layer and manually added them through my model (VAV or FPB access, larger valve handles, AHU maintenance requirements, etc).
Yes you can in a sort of round-about way. You can edit the "Symbol" block of the mvpart and add the clearances on a "your-clearance-name" layer. After the mvpart is inserted, you can control whether those clearances are displayed by freezing/thawing the "your-clearance-name" layer you created.

Good post Buzz, never knew that to be a possibility. Forgive my misleading post above!
