    Default Duplicate Block Insertion

For some unknown reason, one of the guys in the office is starting to insert blocks from his toolbar and a duplicate block is inserting and a random distance/direction from the original insertion point.

It only happened yesterday with no change to the AutoCAD setup or block folder.

    How curious?

    Any thoughts?

What about the block itself in the problem drawing? Does this problem happen on new drawings on his computer?

Yes, the blocks are inserted on all drawings for the majority/if not all all of the blocks.

The duplicate block inserts at random distances and direction from the inserted block making it near impossible to eradicate duplication......

Is the user inserting the block(s) from a network location, or one that is stored on their local disk?

Is it possible that the block definition was modified within that (set of) drawing(s)?

Have you verified that the source block (on the network?) is as it should be?

Is it possible that there is a (poorly written?) Visual LISP routine, or other customization that is causing the undesired duplication?
