Default Visibility state icon off in left field (AutoCAD LT 2019)

I've created a very basic dynamic block tied with 3 states (words/lines on or off). My problem is that the icon for the visibility state (the little triangle) flies off into left field! Like waaaay off. I've gone in a changed it back several times so it's right next to the block, but it doesn't stay where I put it.

Any ideas? This is getting annoying....

How are you editing the db? in place or the block editor or open the dwg? Is the block's insertion point (0,0) near the block entities? Maybe you could wblock it and attach it here.

I've done it both with the block editor and opening the dwg. The block insertion point is near the block. I just tried opening the file (and then the block editor like it asks) and the visibility triangle is right there by the block like it should be. I closed it, then inserted that block into a blank drawing, click on it and it doesn't have the triangle to change states. These pictures {I hope} explain what I'm talking about. The first one is the block dwg in the editor. The second one is the inserted block and "picked" to show the different aspects. (Note: no visibility triangle). The third is the same inserted block after clicking on the block editor button. You see that that tiny little dot is my symbol and the visibility triangle is waaaaay out there!

Your attachment links are not valid. But you've described it well. Can you attach the block dwg and a blank dwg for testing? Is your installation up to date with the latest patches? 2019.1.2

Any of the AutoCAD LT templates will work for the blank dwg. And I realized that there is more than 3 states... oh well, it still comes in way off base.
    Thanks for your help!

I think it may be that the block was unitless. Try this block. I moved the visibility back. In the block editor, I used the Saveas command in the first ribbon panel. When redefining, I changed the units from unitless to inches.
