How did you start Civil 3d? The shortcut link to start Civil 3D has a /product switch for C3D.
Your screen shot makes it appear you have only started Civil 3D as AutoCAD.
The right pane of the CUI Editor lists the filename of the cui you currently have selected. It should be c3d.cuix if using one without modifications.
If you have a technical question, please find the appropriate forum and ask it there.

I used to have that problem all the time with Map 3D. For some reason, it wouldn't be able to open the Map cuix and it would use the contents of acad.cuix and save it as map cuix. Then, as soon as you try to open acad again, it writes the corrupted cuix to cuix.bak and you lose any hope of recovering the backup. The only solution is to get the original cuix from UserDataCache, which is used to create profiles for new users. The folder is at C:\Program Files\Autodesk\AutoCAD 2025\C3D\UserDataCache\Support.
You can also try the Reset Settings to Default in the Windows Start->Programs (all programs)->Civil3D startup folder,
If the shortcut is missing (Like in lots of Autodesk Products _I wonder why is the case) , you can find it under "C:\ProgramData\Microsoft\Windows\Start Menu\Programs\Autodesk Civil 3D 2025 - English"
It will recover most of the default settings including the menus and CUX files.
If you had custom menus, and toolpalletes, you will loose all those, you have to go as like in New Installation to customize it as you like.Screenshot 2025-04-01 120357.png
Also make sure the "Main Customization File" in Options > Files Tab > Customization Files is pointing to a c3d.cuix
I believe the default location is: C:\Users\<YourUsername>\AppData\Roaming\Autodesk\C3D <version>\enu\Support\c3d.cuix.
