Can't find my newly created custom Tool Palette

I have created a custom tool palette......or so I thought, for an office standard. Through the help menu I learned that in order to do so the information on the palette needed to be stored in a specific place. Based on the help menu, I located the information where I thought was appropriate. Now my tool palette is gone and I am unable to access it. Why? Also, once I am able to relocate the palette how do I get on the server to be accessed by everyone?

I'm not sure of the answer right now, but have you tried the similar threads located at the bottom of this page?
