Sorry Rico, I don't know why this is happening to you...maybe it's the xref gnomes.
I do have a related issue to share, I recently ran into this and had never had it happen before.
I had a plot sheet with an xref, the plot sheet had several viewports in it. I went to plot the drawing and only some of the viewports plotted the xref geometry. The plot preview showed the same thing, only some showing up.
I checked everything I could think of: I made sure the viewport properties were correct, the layer the xref was on was not set to "no plot", made sure the layers in the xref were not set to "no plot", checked maxactvp...(etc). I even copied one of the good viewports and panned over and it stilll wouldn't plot or preview.
What it ended up being was: the xref previously had an "xclip" which was turned off. For some reason it was recognizing the xclip even though it was off and you could see the geometry. To fix it, I just redefined the xclip to the whole xref, and all is fine.
Anyone know why this would happen?
FWIW, in ADT 2006 (working in vanilla 2-d).

Had similar problems (especially with XRefs). The only way I got this to plot was to Recover / Audit each and every DWG file on its own - that's including each DWG xreffed into the DWG with the layout.
It seems as if AC stops plotting when it runs into an incorrect entity. I've found these errors happen in general when you edit XRefs in place.
