Layout names in Sheet List Table

We have a project with multiple (about 10) drawing files and each has 2 - 6 layouts in each. Each layout has a title that designates the phase and sheet number (ie GP-1 for a grading plan). Each layout tab looks something like 5-GP1 for phase 5, sheet GP-1. Is there a way to create a Sheet List Table that displays the Sheet Number, the Sheet Title (OK, these 2 are easy), Drawing File name and Layout Tab Name?

For example, one row of the Sheet List Table would look like this:

Number | Sheet Title | Drawing File Name | Layout Name
GP-1 | Grading Plan | GVW002GP.dwg | 5-GP1

Out-of-the-box it's not possible to do that unless you manually add the layout name to a custom sheet property. But it's possible to program AutoCAD to update the custom sheet property with the name of the layout.
